Service Description
Details about documentation will be given when appointment is made. If walking in, bring identification, any court documentation, and, if possible, medicaid card.
Application/Referral Process
New clients must complete a Mental Health Assessment during a Walk in Intake. Walk in intakes are on a first come first served basis, while space is available. Call for a scheduled intake if necessary. Children must be accompanied by legal guardian.
