Service Description
Details about documentation will be given when appointment is made. If walking in, bring identification, any court documentation, and, if possible, medicaid card.
Application/Referral Process
Intakes are available through walk in on a first come first served basis, or through scheduled appointments. New clients must complete a Mental Health Assessment during intake. Children must be accompanied by legal guardian.
Description
Case management assists eligible clients not only in gaining access to needed medical, social, educational and other services. Also ensures coordination between all agencies and providers involved in a client's treatment.
