Service Description
Income verification, proof of Utah County residency, previous year's tax return and W-2 form, bank statements (3 most recent months).
Application/Referral Process
Fill out an application online or in office. A 30 minute oral health education class is required before a person can be seen by a dentist. Classes are held on the second Wednesday of each month.
