Referral from physician (if required), photo ID for parents/guardians, insurance cards and documentation and a current list of medications
Application/Referral Process
Patients receive one-on-one consultation with an audiologist during the initial office visit. They administer tests to evaluate the condition. Based on the test results, treatment options will be discussed.
