Application, proof of income, first 2 pages of income tax filing, Social Security benefits summary page. Some manufacturers require proof of out of pocket medication cost for Medicare recipients.

Description
Provides medication for low-income or those without insurance. The Coordinator works with manufacturers to provide medications and it usually takes 4 - 6 weeks. The Dispensary of Hope will further assist those with no insurance, no Medicaid, and no Medicare.
