Referral required. Children who have been in a NICU and fall under one of the following: require ECMO, are diagnosed with HIE, have a gestational age less than or equal to 26 weeks, or have a birth weight less than 1250 grams.

Receive a referral from a NICU in the state of Utah or a medical care provider.
Description
Provides follow up care and evaluation for children who have spent time in a NICU. Provides a variety of assessments at intervals from 3 months to 4 years of life. Visits available either virtually or in-person.
