Service Description
Birth Certificate, Immunization Record, Proof of Residency, Preschool Application Form, Preschool Student Information Form, Preschool Emergency Information Form, Preschool USIIS and FERPA Release Form. Forms are available online or in the district office.

Screening on site of registration.
