Service Description
Valid Photo ID (Drivers license, state ID card, or passport), second form of ID (Social Security Card or Birth Certificate), Intake fee, and previous treatment documentation if applicable.
Application/Referral Process
Call or complete intake form online. Must arrive for intake in a state of withdrawal of at least 24 hours (unless pregnant), complete paperwork, a physical examination, drug screening, TB test (or proof of test within past two years), and pregnancy test for females.
