Apply online or visit website to print application. Apply before the date employer sponsored health insurance would begin. If already enrolled in COBRA, apply anytime.
Description
Helps to subsidize monthly health insurance premiums when enrolling in an employer’s health insurance plan or those covered by COBRA. UPP will reimburse up to $300 per adult per month and $180 per child per month. May also receive an additional $20 per child if they are enrolled in dental coverage. UPP will mail a reimbursement check each month.
