Application/Referral Process
Clinic sign in sheets are available M, W, F at 2 pm. Sign in sheets are located on a table near the stairs going own to the basement level of the Ogden Rescue Mission. Appointments are first come, first served.

Supplies necessary medications to those in need. Does not provide or supply narcotics, gabapentin, nor neurontin.
