Service Description
Preferably insurance and form of I.D. If no insurance, proof of income and fill out paperwork for that.
Application/Referral Process
Must fill out intake paperwork and bring into the office. Will then be scheduled for an appointment with a therapist.

Provide rehabilitation to adults with mental illness, promote empowerment through meaningful work opportunities, day programs, development of basic living skills.
