M - F 8:30 am - 4:30 pm with evening and weekend activities. Every other W 8 am - 4 pm. Tour: M, T, Th, F 11 am or 2 pm Orientation: W 10 am - 12 noon
Service Description
A copy of the mental health diagnosis, mental health evaluation, treatment plan, and other pertinent history is required.
Application/Referral Process
Members must be referred by a medical professional, preferably one whose discipline is mental health (Psychiatrists, Social Workers, Psychologists, APRNs). New members must attend a tour and orientation.
