Service Description
A copy of the mental health diagnosis, mental health evaluation, treatment plan, and other pertinent history is required.
Application Process
New clubhouse members must be referred by a medical or mental health professional. Also, new members are required to take a tour of the clubhouse and complete an orientation.
Hours: they are also open Thursday nights 4:30 - 7 pm and every other Saturday from 11 am - 4 pm for social activities that can be found on their website.
