Application/Referral Process
Call or visit the website for application form. All applications must be received in person. Those who are unable to return the application in person or are applying during summer months for Beaver, Delta, Fillmore, or Parowan centers should call the Cedar City office at 1-800-796-6070.
Description
Child development program for low-income children or children with disabilities ages 3 to 5 years old. Includes educational services, health services to ensure that every child is properly immunized, has dental and physical exams and nutrition services.
