Breast Milk Banks
Eligibility
Recipients are any infant who cannot receive his or her own mother's milk. Donor human milk goes to the most medically fragile infants first before being made available to others. Healthy mothers who are willing to donate a minimum of 150 ounces of excess breast milk within 18 months postpartum. Mothers must pass a blood test and screening process. Service Description
To donate, fill out form online or over the phone. To receive donation, bring a prescription from babies doctor and parent consent form.
Application/Referral Process
Call the Mother's Milk Bank or fill out the Become a Donor form online. A staff member will call to do a 15 minute verbal screening over the phone, if eligible an in-depth medical history form will be emailed to the donor, then a donor packet will be sent including a donor consent form and provider forms. The donor will also be sent a packet with tubes for a blood draw and a donor number. Bring in all this information to the nearest collection center.
