Eligibility
Adults living in Utah. Must have one of the following forms to qualify for assistance: most current tax return, most recent paycheck stubs (minimum of three), last 3 months of bank statements, proof of food stamps/state assistance (benefits letter), letter from employer (if paid in cash) or religious leader. Household income must fall within 200% of current Federal Poverty Guidelines. Persons may not have substance use as the primary diagnosis. Person may not have a Severe Personality Disorder. Must not have insurance.
