Serves individuals 18 and older who have an active substance use disorder, must have at least five days of continuous sobriety at the time of admission.
Fees
Vary, accepts Medicaid, private insurance, or self pay. For those who cannot afford treatment, funding may be available through Salt Lake County Substance Abuse grant.

Application/Referral Process
Call to schedule a prescreening assessment with the intake coordinator or complete online screening form. Screening process is a 20 minute interview that is by phone or face to face.
