Eligibility
Individuals and families with/without insurance, and the following circumstances: a) been in contact with someone who texted positive for COVID 19 within the past 14 days; b) traveled to an area with widespread transmission within the past 14 days; c) are immunocompromised; d) are over the age of 65; e) live in a long term facility (ie: homeless shelter, correctional facility, or other shared setting; f) are pregnant.

Prescreening by phone. Insurance information or proof of income, and signed consent form.
Application/Referral Process
Appointment necessary. Call appointment line to schedule.
Description
Curbside testing program for the insured and uninsured community members. Results will be reported to each patient through a phone call from the clinician.
