Definition
Programs that help nonprofit organizations, small businesses and other groups develop written questionnaires or checklists that can form the basis of mail, telephone, e-mail, web-based or other pencil and paper surveys that can be used to evaluate their programs and services, products and/or overall organizational effectiveness and efficiency. Questionnaires are tools for eliciting information that can be tabulated for analysis. Considerations in questionnaire design include the type of information being assessed (people's knowledge, opinions and beliefs, behavior or attributes), wording for questions, type of questions (open-ended versus close-ended) and associated rating scales, format (appearance, length and question order), and pretest and questionnaire administration protocols.
