Provides services to individuals with disabilities including advocacy, independent living skills training, youth transition and nursing home/institution transition, peer support, information and referral services, community integration recreational activities, assistive technology equipment services, limited transportation, and Ticket to Work program. Also, provides transportation services to and from the center and activities for those who have no other alternate transportation.
Advocacy
Description:

Consumer Advocacy involves center staff working with individuals to obtain necessary support services from other community agencies. Community Systems Change Advocacy involves staff, board members, and volunteers initiating activities to advocate social and political change for individuals with disabilities.

Disability Related Center Based Employment
Description:

Provides students with disabilities ages 14-21 opportunities to explore career interests, improve workplace skills, and obtain work experience. 13-week development class is required for all participants.

Eligibility:
High school students with disabilities ages 14-21 years old. Serves those in Juab, Sanpete, Utah, and Wasatch school districts.

Description:

Assists people with disabilities who are living in nursing homes or other types of institutions to relocate into a community-based living environment.

Description:

Description:

Provides temporary durable medical equipment for individuals with disabilities until permanent equipment can be accessed.

Eligibility:
Individuals with a disability and who are in need of medical equipment such as wheelchairs, walkers, shower benches, power chairs, and portable ramps.

Description:

Assistive Technology Equipment Listing Services
Description:

Offers evaluation for the need of equipment for consumers. Also assists with the process of obtaining resources for the purchase or repair of equipment. Equipment can include mobility aids, hearing aids or home modifications.

Open to individuals with any form of disability and who are in need of equipment.

Intake Procedure:
Call to schedule an evaluation with the assistive technology coordinator.

No cost assessment. Financial means test to qualify for available state funding for the purchase of assistive technology.

Description:

Provides long term support to minimize confusion, enhance individual choice, and support informed decision-making in accessing long term support services in the community.

Comprehensive Disability Related Employment Programs
Description:

Offers work incentive counseling, skills testing, resume writing, job search and placement assistance, and post-employment services.

Eligibility:
Individuals with disabilities who are receiving SSI or SSDI. Must be 18-64 years old. Ability 1st Utah Employment Services will determine whether or not a client is a good candidate.
