Individuals fill out paperwork in office. Need to bring insurance card and prescription from doctor.
Fees:
Three types of pumps: Amita Maya Joy, Spectra 2, and Medela. The Amita is usually covered completely by insurance. Prices without insurance are: Medela - $600, Amita - $450, Spectra - $500
