Provides low-income women, infants, and children under 5 with nutrition education, vouchers for nutritious foods, breastfeeding support, a review of immunization records, and referrals to other helpful agencies.

Eligibility:
Pregnant or breastfeeding women, women who have just had a baby, or children less than five years of age. Must also meet income requirements.

Description:

Outpatient and intensive outpatient treatment and counseling for substance abuse disorder with certified counselors and educators.

Eligibility:
Individual must be a resident of the tri-county area of Box Elder, Cache or Rich counties to apply for treatment at a subsidized rate. Must be 18 year of age or have a signed consent for treatment for legal guardian.
