Operates out of 6 different locations around the Bear River area.
Prenatal/Postnatal Home Visitation Programs
Description:

Home visits from qualified health department nurses that can answer questions while providing support, education, and resources. Telehealth visits also available.

Eligibility:
Pregnant women or new mothers with children up to 4 years old. Must be on Medicaid with either the Health Choice plan or Healthy U plan.

Immunizations
Description:

Description:

WIC
Description:

Provides low-income women, infants, and children under 5 with nutrition education, vouchers for nutritious foods, breastfeeding support, a review of immunization records, and referrals to other helpful agencies.

Eligibility:
Pregnant or breastfeeding women, women who have just had a baby, or children less than five years of age. Must also meet income requirements.

Substance Use Disorder Counseling
Description:

Outpatient and intensive outpatient treatment and counseling for substance abuse disorder with certified counselors and educators.

Eligibility:
Individual must be a resident of the tri-county area of Box Elder, Cache or Rich counties to apply for treatment at a subsidized rate. Must be 18 year of age or have a signed consent for treatment for legal guardian.

Child Passenger Safety Seat Inspections
Description:

Car seat safety check to make sure car seats are the correct size and are properly installed.

Birth Certificates
Description:

Provides copies of, and allows changes to, birth certificates in Cache county.

Eligibility:
Vital records are protected private information and are only available to the person listed on the record and the person's parents, current/surviving spouse, siblings, children, grandparents, and grandchildren.

Intake Procedure:

Fees:
$22 for initial certificate, $10 for each additional copy ordered at the same time. Fee waivers may be available.

Sexually Transmitted Disease Screening
Description:

Confidential STD/STI testing. Tests are available for HIV, chlamydia, gonorrhea, syphilis, and hepatitis C. Some tests are also available for home delivery.

Chronic Disease Self Management Programs
Description:

Helps people who may be at risk for prediabetes or diabetes to enroll in the CDC diabetes prevention program.

Eligibility:
Participants must have a BMI > 25, be over 18 years old, and be at risk of prediabetes.
