Box Elder Community Health Center

Provides medical, behavioral health, and pharmacy services to the general population. Offers a sliding fee scale which can provide a discount for those patients who qualify. Accepts most insurance plans including Medicare, Medicaid, CHIP, and PCN, and also has a VIP Discount Health Care Plan for those that do not qualify for the sliding fee scale or have high insurance deductibles.
