Home Nursing
Description:

Delivers a variety of in-home health care services including nurses, home aides, medical social workers, physical/speech/and occupational therapists, IV services, oxygen and respiratory services, and durable medical equipment. Telehealth options available.

Eligibility:
Homebound, physician's orders required.

24/7

Fees:
Covered completely under Medicare. Also accepts private insurance. Assistance funds are available for uninsured individuals.

Hospice Care
Description:

Provides home care by an interdisciplinary team for comfort and support for those with life limiting illnesses. Program emphasizes pain and symptom control, as well as support for the clients and families.

Eligibility:
Terminal status and physicians orders required.

Covered completely under Medicare. Also accepts private insurance. Assistance funds are available for uninsured individuals.

Immunizations
Description:

Provides annual flu shots, Covid-19 shots, and other immunizations for both children and adults.
