DDI Vantage Early Intervention

Provides services for children under the age of 3 who have developmental delays/disabilities and their families by offering developmental assessments, early intervention for children with disabilities/ delays, and home-based parenting education. Provides services out of several locations in Salt Lake, Tooele, and Duchesne counties.

Provides early intervention services for children and their families by offering a full range of services to meet their individual needs.

Eligibility:
Children from birth to 3 years of age who have a developmental delay or disability. Family must live within Granite, Murray, Salt Lake, Tooele, or Duchesne school districts.
Service Hours:
M - F 8:30 am - 4:30 pm
Intake Procedure:
Call the local office or submit a self-referral online.
Fees:
Sliding scale fee on a case by case basis. No cost for families with TANF, WIC, Medicaid, or CHIP.
