Promotes opportunities and provides support for people with disabilities to lead self-determined lives. Support services include housekeeping assistance, home barrier evaluation and removal, adult in home respite care, semi-independent living residences for adults with disabilities, case/care management, independent living skills instruction, and supported living services for adults with disabilities.
Case/Care Management
Description:

Provides Support Coordinators who monitor the health and safety of those with qualifying disabilities. Coordinators also inform persons about available services, and help with the selection of providers whom offer those services and monitor through completion.

Eligibility:
Individuals with qualifying disabilities in one of the three areas listed: intellectual disabilities or related development conditions (autism, down syndrome, genetic disorders, etc.), Acquired Brain Injury (ABI), or physical disabilities involving the functional loss of at least two limbs. Also, must be a resident of Utah and a U.S. citizen or legal permanent resident.

M - F 8 am - 5 pm

Independent Living Skills Instruction
Description:

Provides support, supervision, socialization, personal care, training, and assistance in order to help people live as independently as possible.

Eligibility:
Individuals with qualifying disabilities in one of the three areas listed: intellectual disabilities or related development conditions (autism, down syndrome, genetic disorders, etc.), Acquired Brain Injury (ABI), or physical disabilities involving the functional loss of at least two limbs. Also, must be a resident of Utah and a U.S. citizen or legal permanent resident.

Housekeeping Assistance
Description:

Provides services to help maintain a clean, sanitary, and safe living environment for persons with disabilities who are unable to complete chore activities on their own. Includes heavy household chores such as washing floors, windows, and walls, snow removal, lawn care, tacking down loose rugs and tiles, moving heavy items of furniture in order to provide a safe living environment.

Eligibility:
Individuals with qualifying disabilities in one of the three areas listed: intellectual disabilities or related development conditions (autism, down syndrome, genetic disorders, etc.), Acquired Brain Injury (ABI), or physical disabilities involving the functional loss of at least two limbs. Also, must be a resident of Utah and a U.S. citizen or legal permanent resident.

Home Barrier Evaluation/Removal Services
Description:

Services to allow families to make physical alterations and adaptations to the home as needed to help ensure the health and welfare of the individual.

Eligibility:
Individuals with qualifying disabilities in one of the three areas listed: intellectual disabilities or related development conditions (autism, down syndrome, genetic disorders, etc.), Acquired Brain Injury (ABI), or physical disabilities involving the functional loss of at least two limbs. Also, must be a resident of Utah and a U.S. citizen or legal permanent resident.

Adult In Home Respite Care
Description:

Promotes opportunities and provides support for those who care for people with Disabilities, by providing a trained person to temporarily relieve parent(s) or care givers who provide care to the individual with disabilities.

Individuals with qualifying disabilities in one of the three areas listed: intellectual disabilities or related development conditions (autism, down syndrome, genetic disorders, etc.), Acquired Brain Injury (ABI), or physical disabilities involving the functional loss of at least two limbs. Semi-Independent Living Residences for Adults With Disabilities
Description:

Provides living facilities in a group home or host home for those who need more supervision and support development work and community living skills.

Individuals with qualifying disabilities in one of the three areas listed: intellectual disabilities or related development conditions (autism, down syndrome, genetic disorders, etc.), Acquired Brain Injury (ABI), or physical disabilities involving the functional loss of at least two limbs. Disability Related Transportation
Description:

Provides transportation services to help individuals access healthcare appointments, activities, and other social services. Can include bus passes or enhanced paratransit access.

Family Caregiver Subsidies
Description:

Provides limited funds to individuals who serve as the primary caregiver for a qualifying family member with disabilities.

Eligibility:
Caretakers of individuals with qualifying disabilities in one of the three areas listed: intellectual disabilities or related development conditions (autism, down syndrome, genetic disorders, etc.), Acquired Brain Injury (ABI), or physical disabilities involving the functional loss of at least two limbs. Also, must be a resident of Utah and a U.S. citizen or legal permanent resident.

Developmental Disabilities Behavior Management Programs
Description:

Provides support in addressing behavioral problems for people with disabilities. Personalized consultation is given to families to help with serious or complex behavioral difficulties.

Eligibility:
Individuals with qualifying disabilities in one of the three areas listed: intellectual disabilities or related development conditions (autism, down syndrome, genetic disorders, etc.), Acquired Brain Injury (ABI), or physical disabilities involving the functional loss of at least two limbs.
