Promotes opportunities and provides support for people with disabilities to lead self-determined lives. Support services include housekeeping assistance, home barrier evaluation and removal, adult in home respite care, semi-independent living residences for adults with disabilities, case/care management, independent living skills instruction, and supported living services for adults with disabilities.
Description:

Provides Support Coordinators who monitor the health and safety of those with qualifying disabilities. Coordinators also inform persons about available services, and help with the selection of providers whom offer those services and monitor through completion.

Eligibility:
Individuals with qualifying disabilities in one of the three areas listed: intellectual disabilities or related development conditions (autism, down syndrome, genetic disorders, etc.), Acquired Brain Injury (ABI), or physical disabilities involving the functional loss of at least two limbs. Also, must be a resident of Utah and a U.S. citizen or legal permanent resident.

Description:

Provides support, supervision, socialization, personal care, training, and assistance in order to help people live as independently as possible.

Description:

Provides services to help maintain a clean, sanitary, and safe living environment for persons with disabilities who are unable to complete chore activities on their own. Includes heavy household chores such as washing floors, windows, and walls, snow removal, lawn care, tacking down loose rugs and tiles, moving heavy items of furniture in order to provide a safe living environment.

Services to allow families to make physical alterations and adaptations to the home as needed to help ensure the health and welfare of the individual.
