Navigator Programs
Description:

Certified Application Counselors are available to help clients understand, apply for, and enroll in health coverage through the Marketplace, Medicaid, and CHIP.

Intake Procedure:
New patients walk in to establish care, established patients call or walk in to make an appointment.

Glasses/Contact Lenses
Description:

Can fill contact lens prescriptions and glasses lens prescriptions for the unhoused.

Eligibility:
Individuals experiencing homelessness or with an income less than 200% of Federal Poverty Level. Must have a referral from a doctor on staff.
Service Hours:
2nd Tu of each month 5 - 5:30 pm

Fees:
Sliding fee based on income; all insurance types accepted.

General Counseling Services
Description:

Counseling, crisis intervention, mental health, and substance use disorders for individuals experiencing homelessness. Also provides psychiatrists on staff that can help manage and prescribe medications.

Mental Health Evaluation
Description:

Has limited mental health evaluations. Staffed with APRNs and LCSWs on staff that can provide mental health evaluations under their scope of practice.

Family and Community Medicine
Description:

Provides primary medical care services to patients including acute care, specialty care for specific illnesses, and care management and coordination across the medical team.

Call for appointment if established patient (within the last 3 years), walk ins accepted. New patients are advised to walk in and register.
