Navigator Programs
Description:

Certified Application Counselors are available to help clients understand, apply for, and enroll in health coverage through the Marketplace, Medicaid, and CHIP.

General Counseling Services
Description:

Counseling, crisis intervention, mental health, and substance use for homeless individuals. Also provides psychiatrists on staff that can help manage and prescribe medications.

Mental Health Evaluation
Description:

Has limited mental health evaluations. Staffed with APRNs and LCSWs on staff that can provide mental health evaluations under their scope of practice.

Community Clinics
Description:

A comprehensive medical clinic, providing coordinated primary care, behavioral health, limited dental, and pharmacy services to homeless individuals and families.

Call for appointment if established patient (within the last 3 years), walk ins accepted. New patients are advised to walk in and register.

Non-Emergency Medical Transportation
Description:

Provides transportation assistance to and from the clinic as well as any other medical appointments that individuals might need assistance reaching.
