giv.care provides home- and community-based support services for children and adults with intellectual and developmental disabilities (IDD) and acquired brain injuries (ABI). Services are individualized, person-centered, and designed to help people live as independently and safely as possible in their own homes and communities.
Core Services Offered:
Supported Living Services:
* Support for individuals living in their own homes, with family members, or with roommates
* Assistance with daily living skills, including:
* Personal care and hygiene
* Meal preparation and household tasks
* Medication observation and documentation
* Health and safety monitoring
* Support with community access, transportation, and social engagement
* Skill-building to increase independence, self-advocacy, and adaptive functioning
Supported Living with Natural Supports:
* Reduced support for individuals living with parents or relatives
* Designed for situations where family members provide primary supervision and care
* Focuses on intermittent assistance to:
* Prevent isolation
* Support community participation
* Assist with instrumental activities of daily living (IADLs)
Supported Employment (Individual):
* One-to-one employment supports for individuals seeking competitive, integrated work
* Assistance with obtaining, maintaining, and advancing in employment
Service Model:
* Services are provided under **Home and Community-Based Services (HCBS) waivers**
* All supports follow an individualized **Person-Centered Support Plan (PCSP)**
* Referrals are made through state agencies or support coordinators
* Emphasis on dignity, choice, human rights, and community inclusion
