A community partnership and navigator program that works to improve access to, and coordinate comprehensive health care referrals, for low-income, uninsured residents of Salt Lake County. Related interpretation, translation, and volunteer opportunities are available.

Call with medical need. Call will be returned within 24 hours for screening and intake.
