Description:

Diabetes Self-Management Education & Support (DSMES) helps people with diabetes make better decisions about diabetes, work with their healthcare team to get the support they need and improve their hemoglobin A1C levels, blood pressure, and cholesterol levels. The program helps people with diabetes learn how to eat healthy, increase physical activity, check their blood sugar levels, take medication, cope with the emotional side of diabetes, and reduce the risk of other health problems.

Eligibility:
Program participants must be 18 or older and have a body mass index (BMI) of 24 or greater.
