Eligibility:
Individuals and families at or below 150% of federal poverty guidelines,
Cannot be eligible for Medicare, Medicaid, homeless services, or commercial health insurance.

Tu - W 8 am - 12 noon (primary care) & 12 noon - 5 pm (specialty care)
