Provides a disease prevention program and a chronic disease self management program for Utah residents with diabetes or prediabetes.
Description:

Provides The National Diabetes Prevention Program (National DPP) for people with prediabetes at high risk for developing type 2 diabetes.

Eligibility:
Age 18 and older with a BMI of 25 or higher.
Service Hours:
M - F 8 am - 5 pm

Vary; Medicaid, Medicare, and most health insurance are accepted. Scholarships may be available to reduce the program cost.

Description:

Provides a directory of Diabetes Self-Management, Education, and Support Services (DSMES) for people with diabetes. Program objectives include learning to eat healthy and be active, checking blood sugar regularly, information on diabetes medications, how to cope with the emotional side of diabetes, and how to reduce the risk of other health problems. Also offers workshops on specific topics related to diabetes and other chronic conditions.

Diagnosis of Type 1, Type 2, or Gestational Diabetes.
