Medically Vulnerable People (MVP) Shelter & Program

The MVP program shelter and medical services to seniors, veterans, and the medically frail who are experiencing homelessness.
Description:

The Medically Vulnerable People program provides a non-congregate shelter and wrap-around services for the medically frail, seniors, and veterans who are experiencing homelessness. Wrap-around services for clients staying at the shelter include three meals per day, medical case management, housing case management, an on-site nurse, and EMTs and medical services provided by the Fourth Street Clinic. The program serves up to 168 clients (2 clients per room).

Eligibility:
Seniors, veterans, and the medically frail. MVP is not a skilled nursing facility.
Clients must be able to perform Activities of Daily Living (ADLs), including bathing, dressing, eating, and using the bathroom and manage their own medications.

Intake Procedure:
Not a walk-in facility. Clients may receive a referral by walking into the Weigand Center (Catholic Community Services) M - F 8:30-11:30 am & 12:30 - 3 pm. Referrals can be obtained from the Fourth Street Clinic and other homeless resource centers.
