The MVP program provides interim housing and shelter and medical services to seniors, veterans, and the medically frail who are experiencing homelessness.
Description:

The MVP program provides interim housing and medical services to seniors, veterans and the medically frail who are experiencing homelessness. It provides a safe and supportive environment where individuals can connect with housing services, rehabilitate from illness, and access connections to the necessary health care and care management services to find stability and move toward permanent housing and other long-term solutions.

Eligibility:
Seniors, veterans, and the medically frail who have received a referral from the Fourth Street Clinic, another healthcare provider, or the coordinated access entry line.
Service Hours:
M - F 8 am - 4pm (Intake)
Intake Procedure:
Call the SLCo coordinated entry hotline. This program requires a referral from a healthcare provider or the coordinated entry phone line. The eligibility process takes several days to complete.
