For East Millard County: Pick up an application at the Millard County Public Safety Building, 765 So. Hwy. 99, Ste 3, Fillmore, UT 84631. Call for 2023 dates. If you have questions, call the main line and ask for Leslie.

For West Millard County: Please contact the Delta City offices.
