Provides guardianship and conservatorship services for adults who are unable to make basic life decisions for themselves due to medical conditions.
Eligibility:
Utah residents over the age of 18 who have no family willing to serve as guardian, inability to afford a private guardian, and are deemed to have an incapacity in receiving and evaluating information, making decisions, or providing for basic needs.
Service Hours:
M - F 9 am - 5 pm
Intake Procedure:
Create a referral for the individual with the online form, submitting it via email, mail, or fax.
