Rapid Re-Housing Programs
Description:

The Homeless to Housing Program assists individuals and families who are homeless or about to become homeless. Case managers meet with participants regularly to develop and implement an action plan to secure and maintain permanent housing.

Individuals and families who are homeless or about to be homeless
Residents of Davis and Morgan Counties
Service Hours:
M - F 9 am - 5 pm

Case/Care Management
Description:

Provides family support case management that focuses on preventing crises in at-risk families. Case managers complete weekly visits with families, connecting them to local resources and other community programs.
