Initial assessment to determine a child's developmental delays or disabilities. If determined eligible, child may be referred to programs providing early intervention, nursing, occupational therapy, speech therapy, and physical therapy based on individual needs.
Eligibility:
Children age 0-2 living in Uintah and Dagget Counties who may present with a developmental delay.
Service Hours:
M - F 8 am - 4 pm
