Tuberculosis Screening
Description:

Provides skin tests and readings that can diagnose tuberculosis.

Intake Procedure:
Call for an appointment. The test must be read between 48 - 72 hours afterwards.
Fees:
$25 fee for skin test, $130 for blood test.

Birth Certificates
Description:

Provides copies of, and allows changes to, birth certificates in Salt Lake County.

Eligibility:
Vital records are protected private information and are only available to the person listed on the record and the person's parents, current/surviving spouse, siblings, children, grandparents, and grandchildren.

M - F 8 am - 4:30 pm
Intake Procedure:
Requests can be made online, in-person and through the mail.
Fees:
Fees vary but are usually $10-30. Fee waivers may be available.

Death Certificates
Description:

Marriage Licenses
Description:

Source for retrieving copies or editing official marriage and divorce records.

Sexually Transmitted Disease Screening
Description:

Offers STI & HIV tests. Testing is available for HIV, syphilis, chlamydia, gonorrhea, herpes, trichomoniasis, hep B and hep C, and genital warts, pelvic inflammatory disease/infections. Also provides PrEP treatments.

Eligibility:
Anyone 14 and older. Children are not permitted in the exam rooms or lobby.

M - F 8 am - 5 pm
Intake Procedure:
Walk in permitted, but calling ahead to make an appointment is preferred.
Fees:
HIV testing is at no cost unless it is court ordered. Fees vary by test, but range from $40-$80. Cannot bill health insurance.

Immunizations
Description:

Required and recommended immunizations for children, adolescents, and adults.

Eligibility:

Intake Procedure:
Call to schedule an appointment. Same day appointments are often available.
Fees:
Accepts private or public insurance. Uninsured or under-insured families may qualify for Vaccines for Children (VFC). Fee schedule available online.

WIC
Description:

Program provides low-income women, infants and children under 5 with nutrition education, vouchers for nutritious foods, breastfeeding support, a review of immunization records, and referrals to other helpful agencies.

Eligibility:
Pregnant or breastfeeding woman, woman who has just had a baby, or child less than five years of age who meet income requirements.

M, Tu, Th, F 8 am - 4:30 pm; W 9:30 am - 6:30 pm

Description:

Provides treatment for bacterial STD's including chlamydia, gonorrhea, syphilis, pelvic inflammatory disease/infections as well as treatment for viral HPV.
