Child Passenger Safety Seats

Death Certificates
Description:

Provides copies of, and allows changes to, death certificates in Salt Lake County.

Eligibility:

Description:

Healthy living program that provides information on how to prevent and manage chronic conditions including diabetes, heart disease, cancer, and asthma.

Salt Lake County residents with a chronic health condition.

Immunizations
Description:

Required and recommended immunizations for children, adolescents, and adults.

Eligibility:
Salt Lake County residents
Service Hours:
M, Tu, Th, F 8 am - 5 pm; W 8 am - 6 pm

Call to schedule an appointment. Same day appointments are often available.
Fees:
Accepts private or public insurance. Uninsured or under-insured families may qualify for Vaccines for Children (VFC). Fee schedule available online.

WIC
Description:

Program provides low-income women, infants and children under 5 with nutrition education, vouchers for nutritious foods, breastfeeding support, a review of immunization records, and referrals to other helpful agencies.

Eligibility:
Pregnant or breastfeeding women, women who have just had a baby, or children less than five years of age. Must also meet income requirements.

M, Tu, Th, F 8 am - 5 pm; W 9:30 am - 6:30 pm
