Description:

Provides the choice of community alternatives for technology dependent individuals who would otherwise require placement in a Medicaid nursing facility. Waiver services help promote individuals to remain in their homes or other community-based settings.
TDW is funded to serve approximately 142 individuals. Applicants who meet program criteria will be placed on a waitlist. As spots become available, applicants will be enrolled based on highest confirmed acuity score.

Eligibility:
Be under 21 years of age at the time of admission.
Meet admission criteria for nursing facility care.
Meet financial eligibility requirements for Medicaid, based on applicants’ income and assets.
Have at least once caregiver trained and available to provide care.
Require skilled nursing or rehabilitative services at least five days per week and be dependent on one or more of the following: daily dependence on a mechanical ventilator, daily dependence on Bi-level Positive Airway Pressure (Bi-PAP) for 18 hours or more a day, daily dependence on tracheostomy-based respiratory support, daily dependence on Continuous Positive Airway Pressure (C-PAP) or Bi-PAP or High Flow Nasal Cannula (HFNC) for less than 18 hours per day, or dependence on intravenous administration of nutritional substances or medications through a central line, for a period of at least six months.
