A centralized organization that provides government services to Duchesne, Daggett, and Uintah Counties. Services provided include rental deposit assistance, utility service payment assistance, local bus transit assistance, a weatherization program, long-term care ombudsman programs, general health insurance information/counseling, rent payment assistance, home rehabilitation grants, and homeless motel vouchers.
Description:

Offers one-on-one counseling and assistance to people with Medicare and their families. Counselors help seniors understand Medicare, Medicare Advantage Medigap (Medicare supplemental insurance), and Medicaid benefits.

Description:

Assists with payment of electric and gas utilities for families and individuals in need. The HEAT Program year runs from October 1 - September 30.

Eligibility:
Clients are eligible to receive a benefit once per season (Oct. 1st - Sept. 30th). Total household income at or below 150% of the Federal Poverty Level (FPL). Household is responsible for paying home energy costs, and at least one member of the household must be a U.S. citizen or qualified non-citizen.

Description:

Provides housing rehabilitation funding to eligible residents of Daggett, Duchesne, and Uintah counties. Possible home program projects include window replacement, roof repair, electrical needs, plumbing renovation, HVAC system repair, and ADA improvements.

Eligibility:
The property must be in Daggett, Duchesne, or Uintah County. It must also be an owner-occupied, single-family dwelling and be the homeowner's primary residence. Rehabilitation will address problems that are a health, safety, or physical hazard to occupants and make homes accessible to people with disabilities. Rehabilitation work cannot exceed 50% of the value of the home. (A home needing rehabilitation work over 50% of its value will be referred to the Replacement Home Program). Funding comes from several different grants with varying income eligibility requirements.

Description:

A public transportation system serving the Uintah Basin. The primary purpose of this service is to provide citizens of the Uintah Basin opportunities to commute to work, school, and shopping centers in the region. Routes include travel from Duchesne to Roosevelt, Roosevelt to Vernal, and around Vernal.

Investigates complaints and concerns about nursing homes and residential facilities. Provides information on facilities and financing of long-term care programs. Advocates for all residents of assisted living facilities regardless of age.

Description:

Provides a limited number of motel vouchers to unsheltered individuals and families who have exhausted all options to obtain shelter. Motel stays will not exceed a maximum of 7 nights for households with children, and a maximum of 3 nights for single or adult couples. Clients will be required to complete a personalized case plan to obtain housing. Clients will also be required to meet with their case manager to go over potential employment and housing opportunities
