Navigator Programs
Description:

Take Care Utah certified health access assisters are available to help clients understand, apply for, and enroll in health coverage through the Marketplace, Medicaid, and CHIP. Appointments can be completed virtually, over the phone, or in person at one of the office locations.

Case/Care Management
Description:

Provides case management services to individuals with disabilities, assisting them connect to resources and fill out applications.

Eligibility:
Utah residents 18 or older with a traumatic brain injury, mobility limitation, or an intellectual or developmental disability.

M - F 9 am - 5 pm
Intake Procedure:
Submit a referral through the website. Self-referrals are accepted.
