Provides services to adults with disabilities or behavioral issues requiring a high level of support. Services include semi-independent living residences, disability vocational training, and independent living skills instruction.
Description:

Provides comprehensive support to allow adults living with a disability to live independently. Offers fitness education, culinary instruction, home living skills instruction, and vocational training.

Eligibility:
Adults over 18 who are enrolled in Medicaid and have a developmental disability
Service Hours:
M - F 8 am - 5 pm
Intake Procedure:
Call the primary intake line for an initial assessment.
Fees:
Accepts a variety of public and private insurance, sliding scale fees for uninsured or underinsured individuals.
