Description:

Provides case plans for the evaluation, treatment and/or care of individuals who have mental, emotional or social problems and need assistance in arranging for services.

Eligibility:
Serves patients with Medicaid. Does not accept PCN, CHIP or private insurance. Services and funding may be available for eligible uninsured individuals - call for more information.

Office: M - F 8 am - 5 pm; Walk in intakes: M, Tu, Th 8 - 8:30 am & 12 - 12:30 pm
Intake Procedure:
Walk in at an intake time to secure an intake appointment slot. These are offered on a first come first served basis. New clients must complete a Mental Health Assessment during intake. Children must be accompanied by legal guardian.
Fees:
Traditional and non-traditional medicaid services are covered at 100%. Does not accept PCN, CHIP or private insurance. Funding may be available for eligible uninsured individuals - call for more information.

General Counseling Services
Description:

General Assessment for Substance Use Disorders
Description:

Provides substance abuse assessment and treatment. Classes and outpatient treatment available.

Eligibility:
For adults and adolescents. Accepts court appointed.
Service Hours:
M - F 8 am - 5 pm

$108. Accepts Medicaid. Sliding scale available for those who qualify.
