Description:

The State Health Insurance Assistance Program, or SHIP, is a national program that offers one-on-one health benefits counseling to people with Medicare and their families. Senior Medical Patrol counselors assist beneficiaries and their families to prevent, detect, and report healthcare fraud, errors, and abuse with individuals and give group presentations.

Description:

Utah Caregiver Support Program provides information and support to caregivers of adults 60 years old and older or younger adults with a form of dementia. Services include Adult Daycare, Personal Care Aide, Homemaker Aide, Respite, Support Groups, Education, and Caregiver Counseling. Information and referral are also provided for Grandparents over age 60 who are responsible for raising Grandchildren under the age of 18. Senior Companionship Program can also provide respite care.

Delivers nutritious warm lunch meals to homebound seniors age 60 or older. No deliveries on weekends or holidays, but frozen meals are available in advance for those days if requested.

Description:

Transportation for seniors in Weber County without other means of travel to attend medical appointments, senior centers, or participate in community interests, excluding those requiring medical or assisted transport.

Eligibility:
Age 60 and over in Weber County. Rides only within Weber County. Must be able to get from their residence to the vehicle with minimal assistance.

Description:

A voucher program for seniors and persons with disabilities. Vouchers allow clients to arrange rides to medical appointments with a driver of their choice (friends, family, neighbors) who are then reimbursed for the mileage by UTA.

Screening, eligibility review, advocacy, and support access to community programs. Weber Senior Services also provides a traveling 'road show' to groups in Weber and Morgan counties. Topics include the different services provided by Weber Senior Services and can also include tips for seniors to stay physically, emotionally, and mentally healthy 'How to Stay Healthy'.

Description:

Works on behalf of residents of nursing homes, assisted living facilities, and other residential care facilities licensed by Utah Dept. of Health by voicing and resolving the concerns that elderly residents encounter. The ombudsman helps residents to resolve complaints and concerns and informs residents and their families about the resident's rights.
