Description

Provides case management service from start to finish of a client's treatment. Case managers cultivate a relationship with each client to understand the client's challenges, circumstances, and goals. This allows the case manager to recommend personalized care based on the client's clinical needs. Throughout treatment, the case manager supervises the client's progress and facilitates communication between the client, the client's family, and care professionals. Provided at all Valley Behavioral Health locations.
