The applicant must be responsible for the household utilities. A household member must be a US citizen or qualified alien. The household income must be at or below 150 percent of the federal poverty level.

Photo ID for primary applicant, Social Security cards for everyone in the household, most recent utility bills, proof of ALL household income from previous month. Optional documents which increase HEAT benefit: proof of medical expenses paid the previous
