Eligibility
CHAP is for Utah children under the age of six who have been diagnosed as having hearing loss who are ineligible for hearing aids through Medicaid or private insurance and are at or below 300% of the federal poverty level.
HARP has the same eligibility criteria, but for children age six and older.
