Improve the health and quality of life for children and their families, through case/care management, care coordination referrals, hearing aids, early intervention, and audiological evaluations.
Description:

Provides diagnostic infant audiological evaluations, i.e. Auditory Brainstem Response (ABR) testing, via tele-audiology at several locations across the state.

Eligibility:
Infants who have failed newborn hearing screening who are unable to complete the ABR testing due to cost or distance may be eligible for this tele-audiology service.

Tele-ABRs are provided by Utah Department of Health Doctors of Audiology at no cost to families as a public health service.

Description:

Improve the health and quality of life for children with special needs, and their families, by providing hearing aids.

Eligibility:
CHAP is for Utah children under the age of 18 who have been diagnosed as having hearing loss, who are ineligible for hearing aids through Medicaid or private insurance, and are at or below 300% of the federal poverty level.

Description:

Provides care coordination to children, youth, and young adults around the state who have special healthcare needs. Two programs help these individuals, and their families connect with supports, services, and specialists that may enhance the individual's health and well-being. Also provides a transition to adult medicine support and planning for adolescents and young adults. Operates through the Intergrated Services Program and the more wrap-around Intensive Care Coordination program.

Description:

Baby Watch Early Intervention Program (BWEIP) provides support and services to families with children who have developmental delays or disabilities. Early Intervention services are provided through a family coaching model, which focuses on helping children meet goals in all areas of development.

Eligibility:
Automatically qualify if have documented medical diagnosis that meets criteria. Children ages birth - 3 years old.

Description:

The Care Coordinator will act as a central point of contact for parents with children who are seeking information, resources, and referrals for early childhood services that may be an option for them. A care coordinator will provide support through a process that helps families identify and address the needs of young children residing in the home.

Eligibility:
Child does not need to have special health care needs. children ages birth - 8.
