Eligibility
Applicants must be 18 or younger; have 3 separate specialty physicians (must be separate fields, multiple cardiologists do not count); must have seen all 3 specialties in the last 24 months; medical complexity that involves 3 or more organ systems; not meeting age-appropriate activities of daily living; meet minimum score of medical complexity based on combination of device-based supports, medical therapies/treatments, and frequent need for medical intervention; SSI Disability Designation through the SSA or a disability determination by the State Medical Review Board (this will be coordinated as part of the application process).

Description
The Medically Complex Children’s Waiver (MCCW) offers supportive services to the parents and caregivers of medically complex children to promote independence and integration. Application accepted on an on-going basis. Funded to provide services to approximately 900 children. All eligible applications are placed on a waitlist. As spots become available, applicants will be enrolled based on highest confirmed acuity score.
